The SKIP Flu immunization clinic will be offered at Tri County on Monday, October 21. The consent form below needs to be filled out and signed in order for anyone to get a shot. If you have insurance, bring a photocopy of the card with the form. Every student who turns in the Consent Form and gets the flu shot at the clinic will be entered into a drawing for a $20 gift card.
