All orders must include payment. If you have more than one student please complete a separate order form for each student. You may write one check for all of your students. Make checks payable to Images Uniquely Yours Photos by Liz. There will be a $30.00 charge for returned checks.

Return your order with payment to Mrs. Thimm by November 16, 2020. Orders will be picked up on November 16, 2020. Photos will be returned to school.

Photos ordered after November 16, 2020 require a $6.00 late fee.

20% of the sales will be returned to the Tri County Yearbook.
